Pain triggers a protective contraction of the pelvic muscles (hypertonia), which can tighten around the nerve and sustain the pain; stress increases this contraction and your alertness to pain. A seat that forces you to hold yourself up or that locks the pelvis in place also sustains it.

Where does the contraction come from?

First from the pain itself: the body protects the area by contracting. In the pudendal neuralgia cases we have personally assessed, pelvic floor hypertonia has been a consistent clinical observation. Stress, lack of sleep and fear of pain add to it. The pain is not “in your head”: the contraction is mechanical and can be looked for during an examination by a professional.

What cycle sets in?

Step

Effect

Pain

Protective contraction

Contraction

The nerve is tightened from the inside (Alcock’s canal)

Stress, alertness

Contraction and pain perception increase

A rigid or unstable seat

You hold yourself up, you do not relax

Back to the start

More pain

What helps you relax?

Relaxation work guided by a pelvic floor physical therapist, diaphragmatic breathing, gentle activity, and a seat that does not force you to hold yourself up. Strengthening the pelvic floor without an assessment can be counterproductive when there is hypertonia.

What about your seat?

It can force you to hold yourself up, which sustains the contraction, or spare you that effort. A seat that offers constant balance and allows small movements spares you the effort of holding yourself up; static foam does not move, and an unstable seat forces you to hold yourself back. → Which cushion for pudendal neuralgia?

What other questions do people ask?

Can stress cause pudendal neuralgia? No, but it sustains the symptoms.

Should you strengthen your pelvic floor? Not without an assessment: when there is hypertonia, the work first focuses on relaxation.

Publisher: Blue Portance.
