The pain builds up over time and often eases when standing or lying down.

Why does the seat press on the perineum?

Part of your weight passes through the tissues of the perineum, especially on a soft seat you sink into or when the pelvis tilts backward. This is the only one of the five stresses that offloading removes.

Why can it hurt even when nothing touches the area?

The pudendal nerve runs through Alcock’s canal, which is formed by the fascia of the obturator internus muscle. When this muscle and the pelvic floor contract to protect the area, they can tighten around the nerve from the inside. In the pudendal neuralgia cases we have personally assessed, pelvic floor hypertonia has been a consistent clinical observation. Offloading or foam does not act on it; what matters is not to sustain it.

Why does sitting still become painful?

When you sit without moving, the same tissues stay compressed and lose their gliding. Fascia is innervated (Suarez-Rodriguez et al., 2022) and its cells respond to mechanical stress (mechanotransduction). In our files, prolonged immobility makes the pain worse for 9 people out of 69. → Fascia, micro-movements and variability

Why does a seat that is too soft or too hard hurt?

On a seat that is too soft or unstable, you hold yourself up; on a seat that is too hard, you protect yourself. In both cases, contraction increases. 10 people out of 69 tolerate neither soft nor firm seats.

What if it hurts more on one side?

This is common: 36 of the 58 people who answered this question in our files have pain on one side, or stronger on one side. To avoid that side, some people sit “on one buttock”; the other side then carries more. A seat made of a single block does not, by design, offer a different response for the right and left sides.

What makes it worse, what eases it?

Makes it worse

Eases it

A soft seat you sink into

Standing up, lying down

A hard seat that concentrates pressure

Alternating sitting, standing and lying down

Pelvis tilted backward

An open seat (toilet seat) for some people

Long drives, a fixed position

Small, slow movements of the pelvis

For some people, walking

For others, a few minutes of walking

Sitting lopsided

A seat that does not force you to hold yourself up

What if offloading is not enough?

When offloading alone is no longer enough (pain that builds up over time, stronger on one side, or tensing up to stay upright), the question becomes: is there a seat that removes contact where needed, lets each side respond differently and keeps its balance during adjustments? This is the principle of balanced dynamic seats on independent supports, of which Aporia® is one example; the selection guide explains when to consider it and when it is not suitable.

What other questions do people ask?

Should you avoid sitting? No, unless your doctor advises it: break up your sitting time, adapt your seat, and do not stay in a position that clearly makes the pain build up.

Is a firmer seat better? No: it concentrates pressure. The choice is about support without hard spots, not about hardness. → Which cushion for pudendal neuralgia?

Publisher: Blue Portance.
